4 hour Online Adult Abbreviated Course

PLEASE READ THIS INFORMATION CAREFULLY!!

COURSE LICENSURE

Costech Technologies, Inc. - is licensed by the "Department of Public Safety" through the Driver Training Program Office, 1970 West Broad Street, Columbus, Ohio 43223.

Costech Technologies Inc, hereafter referred to as the COURSE PROVIDER.

To be eligible to enroll for Ohio Adult online driver education course, you must:

• Be at least 18 years old
• Have access to the Internet and hardware (see below for details)
• Complete the online application form on the next page. Make sure all is accurate.
• Write down security questions.
• Pay the fee $55.95 (audio available)
• Verification Process–Registration information page
  In the state of Ohio, each person’s validity must be verified through a 3rd party identification database, If over 18 years old, the student’s information must be filled out correctly, if not then an affidavit form must be filled out, signed, and stamped by a notary, then scanned and uploaded to our system.
• Students can not start the course till the information has been verified

Enrolled students must agree to the following:

• I must read every page of the online textbook (monitored)
• I must complete all units in order, starting at unit one, there are 7 units.
• I must have a 75% proficiency on the tests before moving on to the next lesson or unit (allowed only two attempts per day.)
• I have met the minimum of 4 hrs of coursework.
• Each student can only complete 6 hours of log in work each day, then they will be logged out till the next day ( starting at midnight)
• In order to receive the certificate for the course, enrolled students must pass all tests with at least a score of 75%.
• Final exam will only be made available after all the above criteria have been met.
• Student will have 3 tries to Pass the Final exam.  After 1st try- student will be locked out for 24 hrs before trying a 2nd time.
• Per DPS regulations, all course work including the final exam must be completed within 3 months ( 90 days), from the first date of starting the first page of the Book/Course.  If it’s not completed in 3- months ( 90 days), they must take the course over again.

CANCELLATION AND REFUND POLICIES

Course payment goes to:  Costech Technologies or Costech USA

I must notify Costech Technologies Inc. (SEE Contact Information) of my decision NOT to continue if I request a refund (only applicable within 7 days after registration).

PayPal Payment only accepts Credit Cards, Not Debit cards

Refund Policy

I must notify Costech Technologies Inc. (SEE Contact Information) of my decision NOT to continue if you request a refund.

A full refund minus a $10-dollar administration fee will be made to any student who cancels their contract within 7 days of registration for the course and prior to completing a unit.

HARDWARE AND SOFTWARE

The COURSE PROVIDER prescribes minimum hardware and software requirements for completing the course. Minimum browser requirements advise the use of Google Chrome be used for the Online Course. The minimum hardware required is such that it should be able to support the software configuration listed above. The course requires the use of a pointing device such as a mouse. The course requires that the hardware be equipped with speakers. All students will go through the software evaluation after registration, but prior to the start of the course.

COMPLETION AND CERTIFICATION POLICY

The certificates are verified and issued by a Person, ( Monday to Friday 9 am - 5 pm) not automated.  As per New State Regulation, Certificates can now be Emailed.  Completion Certificates are issued and emailed to the student, not before 24-72 hrs after completion time.  Look in Spam/Junk mail if not found in your Inbox.  Use an email that is not blocked by a Company.  If student requests for their certificate UNDER 24 HRS from Completion time an Expedition fee will be asked to be paid before ( fee of $25) .  Contact support@costech.com 

Print the Certificate then:  Contact the Driving School directly to Book your Behind the Wheel Lessons.

DUPLICATE/REPLACEMENT CERTIFICATES

The student is solely responsible for any errors in personal information.
Any duplicate request for a corrected certificate will be charged $8, .

If the student has not received the Certificate within 72 hrs, Check your Junk/Spam email box.  If still not received Contact: support@costech.com

Replacement/Duplicate: Requests after 7-10 days from Completion time will need to pay a fee of $8

When communicating or corresponding with Costech administrators, teachers and support personnel that I must NOT use abusive or offensive language. If I use abusive or offensive language, I may be suspended or removed from this course and I will not receive my Certificate of Completion or tuition refund.
